Liturgy Planning Forms

First, contact the Office of the Bishop to request a liturgy.

Once the liturgy is approved, a Liturgy Preparation Form (LPF) must be completed and submitted 90 days before the event to the Worship Office for review.  It must also include the Universal Prayer.

Confirmation Masses are scheduled through the Office of the Bishop.

A Confirmation Liturgy Preparation Form (LPF) must be completed. The Office of Worship will email you an LPF that has the Mass that will be celebrated and the readings. The parish will then provide all the other required information and return the form, via email, to the Office of Worship.
